As a woman over 40, I'm wondering about medical facilities in USVI. Do you have to go back to the mainland for things like mamograms and colonoscopies?

Yes they have these types of services here. However my wife always has these things done "back home" in Akron, Ohio since (a) her doctor is there and (b) she goes to Ohio 2x/year.

On St Thomas, mammograms are done at St Thomas Radiology in the Paragon Building. Dr L Goldman does colonoscopies on a outpatient basis at SRMC.
